First Alert 4 has received notifications from the Medicare & Medicaid Services Center (CMS), published on April 2nd.
According to the notice, the facility was unable to comply with multiple Medicare and Medicaid participation requirements significantly. The agency cited abuse, neglect, exploitation, quality of life and quality of care.
First Alert 4 visited the facility on Monday. The front door of the building was locked. A letter on the door confirmed the closure.
We reached out to the number left at the door to learn more about what led to the closure and where residents were moved.
