
Oracle today announced the general availability of Oracle Health Community Care, a cloud-based mobile extension to Oracle Health Foundation Electronic Health Record (EHR). The new product allows caregivers to safely access and update comprehensive patient charts regardless of network connection, allowing nurses, therapists, dietitians, social workers and physicians to have the information they need to perform in-home patient visits, provide care at mobile clinics, and provide emergency interventions.

Expanding access to care
Due to factors such as rising healthcare costs, aging populations, and lack of staffing, more services have shifted from hospitals to community settings due to preferences for home care. However, unreliable network connectivity in some areas can limit access to comprehensive patient data, which hinders the quality and timeliness of care. Oracle Health Community care allows for limited connectivity or access details on patient charts, keep track of patient encounters, and continue to provide the whole-person approach needed for treatment. Unlike other mobile solutions available in the industry, when a device using Oracle Health Community Care is reconnected to the network, information added to the patient record offline will be synchronized with the EHR to update the master record.
For example, if a patient is discharged from a hospital to a home care environment, Oracle Health Community Care allows clinicians to safely download the latest patient information from the Oracle Health Foundation EHR via mobile devices and continue to maintain access to those records even when internet connection is lost. Because you are visiting patients at home, you can refresh your patient records and then upload them to the EHR when the device is reconnected to the network, allowing the entire care team to display closer to real-time care activities. With Oracle Health Community Care, clinicians can efficiently record patient assessments, patient orders, or other changes in condition directly within mobile solutions. This timely access to this important information allows caregivers to provide personalized care tailors to each patient's unique needs.
Additionally, Oracle Health Community Care integrates with mobile device mapping tools to enable practitioners to view details of their neighborhood and patient homes and to encourage home visits. Practitioners can also call and message directly with the patient's primary care provider to connect all care team members and to obtain the necessary guidance on patient care.
