BBCA ‘LANDMARK’ contract has been signed to provide Jersey’s first island-wide remote health monitoring service.
Graphnet Health has been selected by Family Nursing & Home Care (FNHC) in Jersey to use its monitoring solution to track the health of vulnerable Islanders at home and in the community.
The initiative is part of an £800,000 CareTech grant awarded to FNHC to roll out a digital system for older people on the island.
Family Nursing & Home Care chief executive Rosemary Finley said the project would provide a “powerful way” to look after people at home.
“As a local nursing organization, we see first-hand the impact that frailty, falls, poorly managed wounds and long-term illnesses such as diabetes have on patients and their families.
“With funding from the CareTech Challenge, we will be able to use Graphnet’s remote monitoring technology to detect warning signs earlier, intervene sooner, and help more Islanders stay independent, safe and healthy for longer.”
