
Ignored and unsafe: New research reveals flaws in intrusive nursing homes across the United States
A new study by Bader Scott ranked states based on data from nursing facilities reporting defects for assessing quality of care. Many states fail to provide the quality care that older loved ones deserve, causing families to worry about their safety and well-being. California, Washington and New Mexico are among the states with the worst nursing homes, a new ranking reveals.
The study, conducted by injury lawyer Bader Scott, examined data on nursing facilities that mentioned defects ranked in nursing home quality.
When it comes to taking care of our elderly parents and grandparents, the quality of the nursing home they live in is important. Unfortunately, new research shows that many states are insufficient to provide the level of care and safety our loved ones deserve. In some states, almost all nursing homes report significant flaws in areas such as quality of care, resident rights and pharmacy services.
Families should be particularly concerned that the flaws are widespread and serious, and that vulnerable elderly people are at risk of negligence and harm. In these areas, residents may not receive the necessary attention or care. Family wonders if their loved ones are in really good hands.
Many states also report surprisingly high percentages of defects, with many nursing homes showing serious shortcomings in key services such as nutrition and infection control. With so many facilities need improvement, it is important for families to be vigilant and aware of where their loved ones live.
By highlighting these troubling statistics, the goal is to encourage better care and more surveillance across the country to ensure that elders have the dignity and respect they deserve in their final year.
In this study, we calculated weighted average deficit rates for each state using the number of facilities that achieved a lack of various categories. The higher the weighted average rate, the lower the quality of the nursing home.
The deficiency categories were as follows:

California has the worst nursing homes, with an average shortage of 63.6%. 0% of the state's nursing facilities did not record defects, but more than a quarter (26%) had serious defects.
In California, 85% of nursing homes had defective quality of care, and 83% had defects in pharmacy services and food and nutrition services.
Washington is the most problematic nursing home that comes close to two seconds, with an average deficiency rate of 63.1%. Of the state's 198 nursing homes, 98% had at least some recorded defects. The state also recorded an average of 15.3 defects per nursing facility.
The majority of nursing homes in Washington (89%) had defective quality of care, with 76% having defective residence rights.
New Mexico ranked third, with an average defect percentage of 62.5%. In this state, 100% of nursing facilities had several recorded defects, with 41% having serious defects. 85% of New Mexico nursing facilities had flaws in comprehensive resident-centered care plans.
The fourth is West Virginia, with an average shortfall of 57.5%. Of the 123 nursing facilities in the state, the average number of shortages per facility is 12.4. Data also showed that 89% of facilities had defective quality of care.
Maryland was fifth in its state, with an average shortfall of 57.3%. The state, in collaboration with New Mexico, has the highest average number of shortages per top 10 certified nursing facilities (15.4).
In Maryland, over three-quarters of nursing facilities (78%) lacked the rights of residents, while 76% lacked the quality of care.
Sixth is Nevada, with an average shortfall of 57.2%. In this state, 18% of nursing facilities are seriously defective. 84% of nursing facilities lacked quality of care, and 81% had a shortage of pharmacy services.
Oregon ranks 7th, with an average defect rate of 56.1%. Oregon had the lowest average shortfall per certified nursing facility among the top 10 states, with 10.8 per facility.
We also recorded 2% of nursing facilities that were not defective. However, a majority (84%) of nursing facilities in Oregon recorded a lack of quality of care.
Hawaii is number eighth on this list, with an average deficit rate of 55.9%. Hawaii has the lowest number of nursing facilities in the top 10 states (43), with an average number of defects per facility of 11.7. In this state, 70% of nursing facilities lacked quality of care.
The ninth is Missouri, with an average shortfall of 55.2%. Almost a quarter (24%) of nursing homes in this state have significant defects, with an average number of shortages per nursing facility of 11.8. 74% of facilities in the state lack the quality of care, while 68% lack the rights of residents.
Michigan is number 10. The state's shortfall is 55.1%. The proportion of nursing facilities that are seriously defective in this state is 48%, the highest percentage of the top 10 states. 88% of nursing facilities in Michigan had defective quality of care, and 64% lacked rights for residents.

A spokesman for Bader Scott commented on the study: The findings are of concern, particularly in states such as California, Washington and New Mexico, with almost all facilities reporting defects. Some of it was serious.

Methodology:
Data were collected from tables referring to “defects” and used to rank states for nursing home quality. Calculate the weighted average deficit rate for each state using the number of facilities that have achieved a shortfall in each category.
Percentages were averaged with the following weightings to calculate the rank of each state:
