Froedtert Holy Family Home Health Care will be integrated with another home care service in the future.
Froedtert and ThedaCare At Home will partner on January 1 to enhance access to care and provide additional services such as hospice and palliative care to more people and patients in surrounding areas, including Manitowoc, Sheboygan and Kewaunee counties.
Froedtert Holdy Family Memorial Hospital Vice President Brian Graf explains that the demand for home care services is rapidly increasing due to an aging population, changing patient preferences, and technological advancements.
Medical organizations explain that by 2030, one in five Americans is expected to be 65 or older.
That number could rise to 95 million by 2060.
Home health care is an alternative to long-term hospital or nursing home stays and includes care for most age groups.
Current patients will be notified and given appropriate details regarding future transitions.
