Home health care, home care, and all-inclusive care programs for the seniors (PACE) landscape saw a fresh deal this month.
H/CARE adds American Premier to the platform
H/CARE is growing its platform and expanding its Arizona footprint through partnerships with American Premier Home Health, Hospice and Palliative Care.
“Phoenix is one of the most dynamic aging markets in the country, and the American premier has acquired its position as one of the region's most trusted names,” said Ryan Haller, CEO and co-founder of H/CARE in a statement. “This collaboration will enable us to deepen our investment in Arizona and bring a unique model of support, technology-ready and clinician first care to a larger population.”
Through the partnership, H/CARE oversees all care operations. American Premier integrates with H/Care's National Hub-and Spoke platform.

Finally, American Premier is a Phoenix-based home care provider.

Home locations unfold four new franchise locations and enter a new state
The home location has announced locations for four new franchises, which will open in June. The company currently serves in Weston, Florida. Reston, Virginia. Raleigh, North Carolina. Yoruba Linda, California.

The home location is a senior care franchise founded in 2012.

Valley Health Partners with Inbound Health launches home program at hospitals
Valley Health System is working with Inbound Health to launch a hospital-based in-home program. The new program is expected to be released this summer.

The hospital expands the trustworthy care that our community expects beyond hospital barriers, providing the safe and high quality services that are most needed,” said Dr. Robert Brenner, president and CEO of Valley Health Systems, in a statement.
Valley Health System is a healthcare system operated in northern New Jersey. The health system includes Valley Hospital, a 385-bed nonprofit acute care hospital, and Valley Home Care, a home care and hospice agency.
Inbound Health is an activation platform that helps health systems and health plans develop at-home and SNF-AT-AT-Home programs.
As part of the program, eligible patients will have access to daily clinical assessments from nurses and clinicians, 24/7 monitoring, medical supplies and home visits.
FreedomCare expands in the Midwest
Home care company FreedomCare has launched services in Illinois and Ohio.
In Illinois, the company launched its services through its Home Services Program (HSP). This allows Medicaid beneficiaries to access personal care services from informal caregivers. This often means friends and family.
“All Medicaid members in Illinois deserve timely, caring and reliable care,” Caitlin Griffin, Illinois Operations Director at FreedomCare, said in a statement. “From downtown Chicago to Cook County and beyond, we are honored to be able to bring families together and provide domestic care services that promote independence in the home.”
In Ohio, the launch of FreedomCare's structured family care program will provide eligible Medicaid beneficiaries with access to home care from informal caregivers.
“All Medicaid members in Ohio deserve timely, caring and reliable care,” Griffin said in a statement. “We are honored to provide in-home care services to families in the Cleveland area. With over 72,000 families already supported nationwide, FreedomCare continues to bring the dignity and respect they deserve to Ohio families.”
Through these programs, FreedomCare trains these informal caregivers, processes paperwork and provides additional, ongoing support.
The pace program Carolina SeniorCare will be released
Carolina Senior Care, a new North Carolina-based pace program, has opened its doors and started offering services.
The organization serves seniors over the age of 55 who live in Beaufort, Carterlett, Craven, Jones, Lenore, Onslow and Pamlico counties.
Organizational services include physician care, nursing services, visual care, dental care, physical therapy, occupational therapy, speech rehabilitation therapy, and preventive services.
“There's no shortage of ideas in Newbern,” Mayor of Newbern Jeffrey Odham said in a statement. “Everyone wants to see Krispy (Cream) or Cracker Barrels. We love those things. We want them to come here too, but what's special about this facility is that it actually meets your needs. We don't necessarily need those restaurants.
