Warren, 55, of Michigan, faces charges after the patient died at an Oakland County nursing home and rehabilitation center.

Antonio Theraon is charged with one count of placing misleading or inaccurate information in a medical record or chart, and one count of unfair practices in a medical professional, a felony charge.

Michigan Attorney General Dana Nessel's Healthcare Fraud Division is handling the case.

Theraon is charged following a case in which he was employed as a nurse at Mission Point, a nursing home in Madison Heights, on December 13, 2023. The disabled resident reportedly hit his head on the floor. Authorities accuse patients of giving controlled sedatives before the fall without proper permission. The resident later died of injuries in the fall. Theraon is also accused of misdocumenting follow-up care that was not provided.

“Illegal behavior by healthcare providers is deeply concerned, especially when we directly compromise patient safety,” Nessel said in a news release. “Patients, their families, and other healthcare providers trust nurses to fulfill important duties within the boundaries of professional standards and law. Falsifying records or administering medications in unprescribed ways can lead to serious harm to patients.

Madison Heights police initially investigated the incident.

Theraon will hold a potential cause meeting on March 19th and a preliminary examination will be held on April 2nd.
